Proposed language to add to front page of the "EXIT TERMINATION FORM"

If NetID privileges need to be extended, faculty must notify Chair and cc: <ChemIT@cornell.edu>. For what to write (language suggestions and data required), please see <ChemIT wiki page such-and-such>.

A specific end-date is required, with a maximum duration of one year extension at a time, with the option to renew annually.

Request can be processed before staff member leaves to prevent service interruption.

By default, NetID extension provides departing staff with the following Cornell privileges:

  • Continuation of Cornell email, full storage or simple forwarding and "sending as".
  • Access to VPN so staff can continue to access authorized CCB research servers.
    • NOTE: If this is the reason for a NetID extension, please notify ChemIT that continued authorization to CCB research servers will be required. Thank you.
  • Access to Cornell Library on-line resources.

Workflow idea

1. Prof. signs above form.

2. HR transfers info to CIT form. Can be done well before departure date!

2B. HR emails Chair to sign form and cc's ChemIT as a heads-up.

3. Chair signs form.

4. Form to ChemIT. Gets FAXed in and tracked by ChemIT.

Renewal process

At the beginning of each month, CIT emails Chair with list of NetIDs expiring at end of month.

List sent to ChemIT for processing.

ChemIT notifies sponsoring faculty.

ChemIT renews NetIDs as directed by faculty.
